Each priest is kindly requested to assist the Diocese in its duty to provide for his funeral and burial. The Diocese desires to show its love for the faithful departed by carrying out, so far as possible, the wishes of a deceased priest.
You can submit your funeral plans either by filling out a copy of this fillable pdf and emailing it to Yvette Reategui at yreategui@columbuscatholic.org or printing it out and mailing it “Attn: Yvette Reategui” to the Chancery at 198 East Broad Street, Columbus, OH 43215 or by submitting the electronic form below.
